Real Union, a third tied club, knocked Real Madrid out of the King's Cup on away goals after a last-ditch strike secured a 6-6 aggregate draw.
The Basque club had won the first leg of the last-32 encounter, 3-2, at home in Irun. It trailed 4-2 at the Bernabéu in Madrid on Tuesday when Eneko Romo buried a header past keeper Jerzy Dudek in the 90th minute.
"There was no problem with our attitude," Bernd Schuster, the Madrid coach, said after fielding a team weakened by injuries.
"The problem in recent games is our opponents have had very few chances but have scored from them. The team is producing comebacks but you can't do it every time. It's worrying."
Real Madrid, the reigning Spanish league champion and 17-time cup winner, made a lethargic start and fell behind when Paul Abasolo was left unmarked to score.

But Madrid's defense, which leaked three goals at home to Malaga on Saturday, was caught out again at the end when Romo struck.
FRANCE Lyon was knocked out of the French League Cup after a 3-1 defeat by second-division Metz in the fourth round on Tuesday, exactly a year after their last home defeat.
Lyon, which had not lost at home since a 2-1 defeat by Marseille on Nov. 11 last year, rested several starters including top scorer Karim Benzema for its next league game against Bordeaux.
Metz led 2-0 at the interval after scoring twice against the run of play through Victor Mendy and Sebastien Renouard.
Cris revived Lyon when he headed home a corner just after the break but in the 66th minute Romain Rocchi slipped the ball into an empty goal after Lyon keeper Remy Vercoutre had rushed out.
